A Superannuation Choice Form is a form that is used by:
- an employee would like to have their employer contributions directed to a particular superannuation fund and are eligible to under the terms of their current employment; or
- an employer with employees who are eligible to choose where they would like employer superannuation contributions to be made.
The Superannuation Choice Form can determine where any of the following contributions are made:
- SG Contributions
- Salary Sacrifice Contributions
- Other Employer Contributions
